Dear Teachers and Administrators: Help Us Help You!
On behalf of the Ohio Department of Education, Hezel Associates asks for your participation in a short, online survey about using data. Your input will help with Ohio’s Data Driven Decisions for Academic Achievement (D3A2) statewide initiative, which strives to provide administrators and teachers with easy access to student data and accompanying instructional resources.
We estimate it will take you 15 minutes to complete the survey. The survey and a program description are located at the following Web site. Click on the link below, or cut and paste the entire URL into your browser.
Teachers: http://www.hezel.com/OHteachers
Administrators: http://www.hezel.com/OHadministrators
We would appreciate a response by May 9, 2008.
Please share this survey link with your colleagues. Your answers to these questions are most important, and will be kept confidential (used only for the purposes of research for this project). If you have any questions please call us at 800-422-3512 or email Kathy DelSavio at Kathy@Hezel.com.

Would you like to be trained on introducing district staff to the use of D3A2? Another series of D3A2 Train the Trainer Sessions is being offered. Two regional sessions are already scheduled with three more to come. The intended audience includes: ITC staff, ESC staff, SST staff and other professional development agencies and roles. Check back for a session to be held in your area of the state.

D3A2 Implementation Request Form
Districts interested in using the D3A2 should register their district by filling out the D3A2 Implementation Request form.
Please note that submission of an Implementation Request Form will not provide immediate access to the D3A2 system. Please refer to the project status page to find out the current state of district implementation.
